We are seeking a Case Manager who will be responsible for Person-Centered Planning to develop an Individual Plan of Service. Provide on-going linking, coordinating, and monitoring of services.
00 am to 5:00 pm. There are no weekends, on call, or holiday requirements for this position. The employee in this position will work a hybrid schedule, working in office at 790 Fuller Ave NE Grand Rapids, Michigan 49503 and within the Kent County community.
