The Patient Care Coordinator manages engagement and coordination for perinatal and postpartum patients, facilitating access to healthcare providers and addressing social determinants of health. They are responsible for patient education, EMR documentation, and supporting quality improvement initiatives to improve maternal health outcomes.
Requires a high school diploma and 2-4 years of healthcare experience in care coordination or patient navigation, with a preference for degrees in healthcare or LPN certification. Experience with perinatal populations or NICU settings is preferred, along with strong interpersonal skills and the ability to work autonomously.
Patient Engagement & Relationship-Based Support Engage pregnant and postpartum patients across risk levels Conduct outreach via phone, text, video, and in-person visits Explain care management services and support program enrollment Build trust using trauma-informed and culturally responsive communication Care Coordination Assist patients in understanding and navigating health plan benefits Coordinate appointments with OB/GYNs, PCPs, pediatricians, specialists, and behavioral health providers Address social determinants of health including transportation, housing instability, food insecurity, and childcare Collaborate closely with OB/GYN providers and interdisciplinary teams to ensure seamless transitions of care Patient Education & Empowerment Provide education using approved materials Promote self-management and patient empowerment Reinforce postpartum follow-up, well-child visits, and preventive care adherence Documentation & Technology Document all encounters and care plans accurately in the EMR Participate in virtual case reviews and team meetings via Microsoft Outlook and Teams Maintain patient registries and support reporting initiatives Complete administrative duties as assigned Quality & Program Support Contribute to quality improvement initiatives focused on maternal health outcomes Support performance metrics such as postpartum visit completion and preventive care adherence Adhere to organizational policies, care pathways, and safety standards
