Responsibilities
Observe residents continuously, noting changes in mental and physical well‑being; communicate pertinent observations to the supervisor and provide a comprehensive handoff at shift change.
Transcribe and execute physician orders accurately; administer medications and treatments and document in MARs/TARs and electronic records.
Administer oxygen as directed, and perform gastrostomy/jejunostomy feedings per policy; provide skilled wound care including wound vac management.
Supervise LNAs within scope of practice; assign and monitor nursing care, assisting direct-care staff as needed; participate in physician rounds on assigned units.
Manage unit operations during the shift, including admissions, discharges, transfers, antibiotic tracking, and daily rounds for therapy appointments; maintain infection control practices and a clean, hazard-free environment.
Communicate with residents’ families about orders and care plans; prepare and deliver 24-hour unit reports; uphold chain of command and contribute to ongoing staff education and training.

Requirements
Active New Hampshire Licensed Practical Nurse (LPN) license - 3+ years of current nursing experience within the last 6 months
IV certified (must be willing to obtain during onboarding); BLS (AHA) certification
Willingness to float to units with experienced staff; ability to adapt to a rural, small-facility environment
Covid-19 vaccination required (declinations not accepted)
Locational requirement: must reside beyond 50 miles from the facility; travel experience preferred
Two supervisory references from the last 2 years
