Provide telehealth triage services to assess patient needs and determine the urgency of care. Conduct both virtual and in-person assessments while collaborating with medical directors and staff to develop individual care plans.
Requires a Bachelor of Science in Nursing, a valid Texas RN license, and at least 5 years of experience. Must possess BLS certification and be able to pass comprehensive background and drug screenings.
1. Pass a pre-employment drug screen and random drug screens throughout employment. 2. Provide proof of work eligibility status upon request. 3. Pass a pre-employment and biennial criminal background checks. 4. Demonstrate the ability to: a. Work collaboratively with other staff members, service providers and professionals. b. Communicate effectively in writing and verbally in English. c. Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times. d. Maintain computer literacy required to meet the responsibilities of the position. e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team. 5. Demonstrate a working knowledge of all Compass Connections policies and procedures. 6. Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for the following: a. MMR b. Varicella c. Hep B d. Hep A e. Tetanus f. PPD g. Meningococcal (preferred) 7. Must be able to perform physical activity such as extensive walking, stretching, and bending. 8. Must be able to work in a team environment. 9. Must be able to maintain professional people skills, including oral, written, listening, and non-verbal communication skills.
