The Physical Therapist is responsible for evaluating patients and developing tailored treatment plans to enhance mobility and independence for seniors. They must administer therapeutic programs, maintain patient documentation, and supervise Physical Therapy Assistants.
Candidates must hold a valid Arizona Physical Therapist license and a current CPR certification. Two years of home health experience and a background in geriatric care are preferred.
Overview We are seeking a Physical Therapist to provide skilled physical therapy services to patients across Senior Living Communities. In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans. Evaluate physician’s referral and patient’s medical record. Assessing and evaluating therapeutic/rehabilitative/functional status using approved evaluation techniques, tests and observations, and participation in the development of the total plan of care. Evaluates the home environment. Determines priority needs for physical therapy. Directs physical therapy treatment. Reports to physician, patient’s response to treatment and changes in condition. Identifies patient and family/caregiver needs for other home health services and refers as necessary. May train patients in the use of prosthetic device. Administer a prescribed physical therapy program to restore and maintain maximum function and prevent disability using a range of therapeutic modalities. Maintain appropriate documentation in the patient chart, and re‐evaluate patient progress and revise treatment programs when necessary. Communicates physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team. Supervise Physical Therapy Assistants according to Agency policy and state regulations.
Valid Physical Therapist license in Arizona, in good standing. Two years of Home Health experience as a Physical Therapist preferred. Possesses and maintains current CPR certification. Licensed driver with reliable vehicle and current vehicle insurance. Experience in geriatric physical therapy or working with seniors is preferred. Ability to work independently and manage time efficiently. Excellent observation, verbal and written communication skills, problem solving skills, and organizational skills. Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team. Schedule and service areas: PRN to cover the West Valley & East Valley
