Locations
Conduct health assessments and monitor the well-being of supported individuals while coordinating medical appointments. Collaborate with interdisciplinary teams and the Medical Services Team to promote independence and high-quality care.
Requires an active New York State RN license and a minimum of 2 years of nursing experience. Experience with OPWDD is considered a major plus.
