• What you’ll do
• Conduct health assessments and ongoing monitoring for the people we support
• Coordinate and schedule medical appointments and follow-ups
• Partner with our Medical Services Team to ensure seamless, high-quality care
• Collaborate with interdisciplinary teams to support person-centered outcomes
• Advocate for and promote the overall health, wellness, and independence of the people we support

• What You Bring
• Active, valid, NYS RN license
• Minimum 2 years of nursing experience
• Strong organizational and coordination skills
• Passion for community-based care and person-centered support
• OPWDD experience is a major plus
