The Hospice Social Worker provides emotional, social, and practical support to patients and families through counseling and resource coordination. They collaborate with an interdisciplinary team to facilitate end-of-life decision-making and ensure patient dignity.
Candidates must hold a Master's Degree in Social Work from a CSWE accredited program and be licensed in Massachusetts. At least three years of recent experience in acute, long-term, or home care is required, with hospice experience preferred.

As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end-of-life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time. Key Responsibilities Conduct psychosocial assessments of patients and families Provide emotional support, counseling, bereavement and crisis intervention as needed Assist patients and caregivers with advance care planning and end-of-life decision-making Connect families with community resources and supportive services Facilitate discussions regarding patient goals, family dynamics, coping, and bereavement needs Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team Advocate for patient and family needs while promoting dignity, comfort, and quality of life Complete timely and accurate documentation in accordance with hospice standards Reporting Relationship: Executive Director
Masters Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies. Strong communication, counseling, and interpersonal skills Ability to work independently while remaining an engaged member of a multidisciplinary team Compassionate, empathetic, and patient-centered approach to care We are committed to finding someone who shares our values and passion for hospice care, and we are open to creating a role that supports both the needs of our patients and the strengths of the ideal candidate.
