Pagosa Springs Medical Center (PSMC) is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing rehabilitation team. We are open to hiring either a part-time or full-time Occupational Therapist, offering flexibility for the right candidate. This position provides evaluation, treatment, education, and care coordination services for patients with physical, developmental, cognitive, sensory, and functional limitations affecting their ability to perform activities of daily living. The Occupational Therapist develops individualized treatment plans, recommends adaptive equipment and environmental modifications, provides patient and family education, and collaborates with physicians and interdisciplinary care teams to help patients achieve their highest level of independence and function. This position is also responsible for evaluating and treating patients with upper extremity orthopedic conditions involving the hand, wrist, elbow, and shoulder, as well as neurological conditions, including cerebral vascular accidents, across the continuum of care. The Occupational Therapist provides individualized treatment plans focused on restoring function, reducing pain, improving mobility, and helping patients return to daily activities and work-related tasks. Therapeutic interventions may include, but are not limited to, manual therapy, therapeutic exercise, edema management, scar management, range of motion activities, strengthening, functional retraining, and cognitive skills development. Inpatient responsibilities include patient and caregiver education, home exercise programs, fall prevention, adaptive techniques, and post-surgical precautions. Why This Opportunity Is Unique Flexible employment options: Part-time or full-time Opportunity to help establish and grow Occupational Therapy services within the community Diverse patient population across orthopedic, neurologic, post-surgical, chronic condition, and functional rehabilitation needs Close collaboration with Physical Therapy, Speech Therapy, providers, and care teams Ability to make a significant impact in a rural healthcare setting Key Responsibilities Patient Evaluation & Treatment Perform comprehensive occupational therapy evaluations Assess physical function, cognition, sensory processing, mobility, and activities of daily living Develop individualized treatment plans with measurable goals and expected outcomes Implement evidence-based treatment interventions designed to maximize independence and function Modify treatment plans based on patient progress and changing needs Patient Education & Care Coordination Educate patients, families, and caregivers regarding diagnoses, treatment plans, adaptive strategies, and home exercise programs Recommend assistive devices, adaptive equipment, and home modifications Coordinate care with physicians, therapists, nurses, case managers, and other healthcare professionals Participate in multidisciplinary care planning and discharge planning activities Functional Rehabilitation Provide interventions addressing: Activities of daily living (ADLs) Instrumental activities of daily living (IADLs) Fine motor skills Cognitive rehabilitation Sensory integration Balance and coordination Adaptive equipment training Home safety and accessibility Assist patients with return-to-work and functional independence goals Documentation & Compliance Complete timely and accurate documentation in the electronic health record Maintain compliance with state, federal, and professional practice standards Support quality improvement and patient safety initiatives Participate in continuing education and professional development QualificationsEducation Master's Degree in Occupational Therapy required Successful completion of the National Board for Certification in Occupational Therapy (NBCOT) examination required Licensure & Certifications Current Occupational Therapist license in the State of Colorado required Current AHA BLS certification for Healthcare Providers required Experience Minimum two (2) years of Occupational Therapy experience preferred but not required New graduates with strong clinical training are encouraged to apply Skills Strong clinical assessment and treatment planning skills Knowledge of occupational therapy treatment techniques and adaptive equipment Excellent communication and patient education abilities Ability to work independently and collaboratively within an interdisciplinary team Basic computer proficiency, including electronic health record documentation and Microsoft Office applications Work Environment This position provides services in both hospital and outpatient rehabilitation settings and may include home assessments or home visits when clinically appropriate. The role requires frequent interaction with patients of all ages and diagnoses and involves lifting, transferring, positioning, and assisting patients during treatment activities. Compensation & Benefits Base Compensation Range: $19.50 - $30.14 hourly, based on years of relevant experience. "Relevant experience" means relevant to the employee's responsibilities set forth in the job description as determined by the HR Manager, CAO, applicable department manager/director and applicable senior leader.
