• How you’ll make an impact in this role Ascension Seton is looking for a APRN or PA for our Ascension Seton Health Clinics.

• Post-Acute and Long Term Care Program provides nursing home visits in the rural community - 2-3 sites Travel required Practice setting: outpatient Shared patient panel Employed position within a nonprofit health system Lead Comprehensive Care Delivery: Direct the treatment and long-term management of diverse health conditions, ensuring every patient receives a personalized and effective care plan.

What minimum requirements you’ll need Licensure / Certification / Registration

Nurse Practitioner credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required. Licensed Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.

Education: Master's degree of Nursing require
