Deliver comprehensive clinical care and chronic condition management to patients through in-home visits and community-based facility assessments. Collaborate with interdisciplinary teams to develop individualized treatment plans and maintain accurate electronic medical records.
Requires an active provider license and certification as a Family NP, Adult-Gerontology NP, or Physician Assistant. Must possess a valid driver's license, reliable transportation, and the ability to pass extensive background checks.

As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
