Perform comprehensive in-home health risk assessments for Medicare, Medicaid, and ACA members to identify care needs and support early intervention. Document findings using clinical templates and coordinate with clinical leadership to improve member outcomes.
Requires an active, unrestricted NP or PA license (or MD in NY) with board certification and current BLS certification. Must possess a valid driver's license, reliable transportation, and the ability to pass comprehensive background checks.

We are seeking In‑Home Assessment Providers (Nurse Practitioners or Physician Assistants) to support our health plan partners by delivering high‑quality, in‑home preventive health assessments.
In addition to this flexible fee‑based model, you’ll have the opportunity to support meaningful health outcomes, improve access to care, and work independently with the full support of a dedicated clinical team.
As an In‑Home Assessment Provider, you will perform comprehensive health risk assessments (HRAs) for Medicare, Medicaid, and ACA members within their homes. These assessments help identify care needs, support early intervention, and improve care coordination.
