Provides outreach and support for consumers in the MFP program to facilitate their transition from skilled nursing facilities to home living. Acts as a liaison between providers, social workers, and state programs while managing transition plans and consumer records.
Requires a High School Diploma with 2 years of human service experience, or a Bachelor's degree in Human Services with 1 year of experience. Must possess a valid driver's license, reliable transportation, and knowledge of community resources.

Requirements: Education: High School Diploma or equivalent; Bachelor’s Degree in Human Service or related field preferred
For attained Bachelor’s Degree, at least one year experience in a human service setting For attained High School Diploma/equivalent, at least 2 years in a human service setting. Thorough knowledge of community resources. Ability to work as part of a team and communicate effectively with consumers, families, providers, and other staff. Flexibility with the ability to prioritize and move from task to task in a dynamic, fast-paced work environment. Working knowledge of Microsoft Office products. Environment Work is performed in various environments including offices, apartment buildings, hospitals, and other locations. Employees may be exposed to adverse driving conditions and are to use caution when making decisions.
Reliable transportation, valid driver’s license, and current automobile insurance.
