The Specialized Care Manager coordinates the transition of individuals from institutional settings back into community-based living. This involves conducting comprehensive assessments, developing person-centered care plans, and providing ongoing case management to ensure stability and independence.
A bachelor's degree in Social Work, Nursing, Psychology, or a related field is required, along with at least two years of experience in case management or healthcare. Candidates must possess a valid driver's license and strong organizational and communication skills.
