Responsible for determining and coordinating safe, appropriate discharge plans for patients in an acute care setting. Collaborates with multiple disciplines and community partners to meet patient needs and regulatory requirements.
Requires a nursing degree, an active Virginia RN license, and AHA BLS certification. Candidates must have at least three years of acute care nursing experience.
Care transitions nurse is responsible for the clinical aspects of determining the appropriate discharge plan for patients in the acute care setting. This member of the care transition team will be responsible for facilitating and coordinating a safe appropriate discharge plan while meeting the patient's individual needs as well as state and federal regulations. This position works collaboratively with multiple other disciplines throughout the organization and community.
Specialist Care Transitions LGH/FT 8-430 with some weekends
