Provide comprehensive nursing care for hospice patients of all ages under the supervision of a registered nurse. Coordinate with an interdisciplinary team to administer medications, collect patient data, and document outcomes in the EMR.
Must be a graduate of an accredited vocational/practical nursing school with a current Virginia LPN license and BLS HCP certification. A valid driver's license is required, and two years of clinical or hospice experience is preferred.

The primary function of the hospice Licensed Practical Nurse (LPN) is to provide nursing care, under the supervision of a registered nurse, for hospice patients of all ages in their place of residence. Patient care is coordinated with the Interdisciplinary Team, patient, family/caregiver, and provider. Primary duties include collecting data, administering medications, documentation in the EMR and performing nursing procedures competently as ordered.
