Your Role as a Home Health Aide (HHA/CNA)

As a Home Health Aide, you will provide essential hands-on care to patients in their homes while supporting the overall plan of care.

• Patient Care
• Assist with personal care, including bathing, grooming, and hygiene
• Support patients with daily living activities while promoting independence and dignity

• Clinical Support
• Observe and report changes in patient condition to the supervising RN
• Follow the established plan of care to ensure safe, effective service delivery

• Documentation & Compliance
• Accurately document each patient visit in accordance with agency policies
• Maintain compliance with all regulatory and company standards

Qualifications

• Required
• Completion of a Home Health Aide training program meeting Medicare Conditions of Participation
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to travel to patient homes throughout the day
• Strong communication and interpersonal skills

• Preferred
• Previous home health experience
• CNA certification
• Experience working with diverse patient populations
