The RN is responsible for case managing and implementing comprehensive home health care plans for a variety of patient populations. This includes performing skilled nursing tasks such as administering IVs, wound care, and providing patient and family education.
Candidates must hold a valid New York State RN license and a Basic Life Support Certification. A minimum of one year of recent hands-on nursing experience and a valid driver's license with reliable transportation are required.
Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment Provide care to the most upmost standard
New York State RN license Minimum of one year of recent hands on experience as a Registered Nurse. Basic Life Support Certification Valid New York State Driver’s License and reliable transportation
