The registered nurse provides and coordinates nursing care for patients at the Kidney Center, focusing on wellness promotion and symptom triage. Responsibilities include documenting individualized care plans and educating patients and families on disease processes and preventative care.
Candidates must be a licensed Registered Nurse in Minnesota with at least one year of kidney transplant coordinator experience. A BSN is preferred, and transplant certification is required within two years of hire.
Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by: Protecting, promoting wellness and optimizing health Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care Alleviating suffering through the treatment of the human response Partnering with the patient, their family and significant others Documenting interventions and an individualized plan of care Educating patient on their disease process Assuring continuity of care Educating patient and family in preventative care which promotes health and well being
Graduation from an accredited school of nursing Possession of a valid license as a Registered Nurse issued by the State of Minnesota One year of current kidney transplant coordinator experience Kidney transplant certification within two years of start date
Renal or transplant exeprience preferred Certification in area of clinical specialty ( Kidney transplant) BSN Preferred. Bachelor of Nursing degree is strongly encouraged. Advancement in academic education will be discussed as part of individual professional development during the annual performance review.
