About the Role

  • The Treatment Nurse - LVN or RN at Deeerbrook plays a critical role in delivering high-quality nursing care to residents within a skilled nursing environment.
  • This position focuses on administering treatments, monitoring patient conditions, and collaborating with interdisciplinary teams to ensure comprehensive care plans are effectively implemented.
  • The nurse will be responsible for maintaining accurate documentation and ensuring compliance with healthcare regulations and facility policies.
  • By providing compassionate and skilled nursing interventions, the LVN contributes to improving patient outcomes and enhancing the overall quality of life for residents.
  • This role requires a dedicated professional who can balance clinical expertise with empathetic patient interaction in a fast-paced healthcare setting.

Minimum Qualifications

  • Valid Licensed Vocational Nurse (LVN) or RN license in TX.
  • Current Basic Life Support (BLS) certification.
  • Minimum of one year of nursing experience in a skilled nursing or long-term care setting.
  • Strong knowledge of nursing principles, treatment procedures, and medication administration.
  • Ability to communicate effectively with patients, families, and healthcare team members.

Preferred Qualifications

  • Experience working in a skilled nursing facility or long-term care environment.
  • Additional certifications related to wound care, IV therapy, or geriatric nursing.
  • Familiarity with electronic health record (EHR) systems.
  • Demonstrated ability to work collaboratively within interdisciplinary teams.
  • Continuing education or specialized training in patient care management.

Responsibilities

  • Administer prescribed treatments and medications according to established protocols and physician orders.
  • Monitor and assess patients’ health status, reporting any changes or concerns promptly to the supervising nurse or physician.
  • Maintain accurate and detailed patient records, including treatment logs, medication administration, and progress notes.
  • Collaborate with healthcare team members to develop, implement, and update individualized care plans.
  • Educate patients and their families on treatment procedures, medication management, and health maintenance.
  • Ensure compliance with all regulatory requirements, facility policies, and safety standards.
  • Assist with admission, transfer, and discharge processes, ensuring continuity of care.
  • Participate in quality improvement initiatives and staff training as required.
