The Step-Down Navigator meets with eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. This role also involves communicating eligibility to senior leadership, collecting and entering relevant data into Midas for reporting, and implementing components of the Step-Down Playbook.
A High School graduation or GED is required for this position, though a Bachelor’s degree in social work or a clinical related mental health field is preferred. Candidates must have a minimum of two years of experience in an inpatient psychiatric/mental health setting and hold current CPR and Verbal Crisis Intervention Certifications.

The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. He/ She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Responsibilities include, but not limited to: Meeting with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment. Communicating eligibility to senior leadership and associated departments. Collecting and entering data specific to eligibility into Midas for trending report purposes. Coordinating between departments to ensure a smooth transition for patients.
