Collaborates with physicians and the interdisciplinary team to implement and adjust treatment plans for advanced heart failure patients. Provides clinical health coaching to assist patients with self-management and coordinates care to reduce readmission risks.
Requires a graduate degree from an accredited School of Nursing and a valid Registered Nurse license in the State of Florida. Candidates must have at least three years of nursing experience in an acute care or ambulatory setting.
Graduate of accredited School of Nursing; licensure to practice as a Registered Nurse by the State of Florida Three years nursing experience in acute care or ambulatory setting. Specialty Area of practice qualifications include graduate of accredited School of Nursing, licensure to practice as a Registered Nurse by the State of Florida.
