The Social Worker functions as a member of a multidisciplinary healthcare team to assess and plan for patients transitioning from acute care settings. Responsibilities include completing psychosocial assessments and coordinating discharge plans and community resource linkages.
Candidates must hold a current Missouri License for LMSW and have a Master's degree in Social Work with a clinical practice specialization. Alternatively, candidates with specific experience in dialysis or transplantation prior to 1976 may also qualify.

Responsible for completing psychosocial assessments, coordinating discharge plans, needs, and arrangements for patients, and for providing information, education, and linkage with appropriate community resources and services. Requirements Current Missouri License for LMSW. Master's degree in Social Work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education OR Served at least 2 years as a social worker, 1 year of which was in a dialysis unit or transplantation program prior to Sept 1, 1976, and has established a consultative relationship with a social worker who qualifies under 494.140 (d)(1).
