The Clinical Pharmacist is responsible for providing safe and appropriate pharmacy services, coordinating patient services with the Nursing Department, monitoring patient progress, and educating various parties on appropriate medication use. Key duties include determining patient suitability for home care, designing and evaluating pharmacy care plans, and preparing and dispensing medications following all policies and regulations.
Candidates must be graduates of an accredited School of Pharmacy with a current, active Pharmacy license. Required experience includes one year in sterile IV admixture, knowledge of compounding/MTM, completion of a residency, or equivalent experience approved by leadership. Basic proficiency in Microsoft Office Suite is also necessary.
Summary: The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of the Option Care, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
