Act as a primary case manager for 3-6 consumers, providing nursing interventions and coordinating medical care. Facilitate health education and manage medications while participating in on-call coverage.
Candidates must have an associate degree in nursing and a valid LPN or RN nursing license. Experience working with adults with mental illness and knowledge of psychiatric and medical medications is required.

Eliot is seeking a strong, experienced, licensed RN, Nurse Case Manager, to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.
The ideal candidate would possess knowledge of psychiatric medications, psychiatric and medical treatment. This person would be comfortable with outreach, case management and providing support in individuals homes.
Monday-Friday 8am to 4pm with rotating 12pm-8pm shift , one weekend per month, rotating holiday shifts, rotating oncall schedule
Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase
Annual Salary $58,073.60—$77,680 USD
