As a Part-Time Medical Social Worker, you’ll be a vital member of our care coordination team, collaborating closely with RN case managers, multidisciplinary clinical teams, and internal and external partners. In this role, you will: Perform comprehensive psychosocial assessments Provide supportive counseling Coordinate care related to discharge planning and service coordination Support adult, geriatric, pediatric, and med/surg populations Work in a fast-paced, patient-centered hospital setting where your expertise truly matters 🌊

Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
