The nurse is responsible for the coordination of total patient care and evaluating the effectiveness of nursing services provided to the patient and family on an ongoing basis. This includes performing all necessary documentation procedures such as admission, transfer, recertification, resumption of care, and discharge Oasis for home care patients.
Candidates must be graduates from an accredited School of Nursing and licensed as a Registered Nurse in the state, with one or more years of experience in community/home health or hospital preferred. A valid driver's license, current CPR (BLS) certification, and knowledge of Medicare and Medicaid guidelines are required.

This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rate Mileage reimbursement for any company travel Medical/Dental/Vision Insurance/Supplemental insurance (begins immediately upon hire) Company 401K with 4% company matching

Graduate from an accredited School of Nursing Licensed in the state as a Registered Nurse One or more years of experience in community/home health agency or hospital preferred Knowledge of Medicare and Medicaid guidelines Criminal background check Must have a current CPR (BLS) certification
Valid driver's license required Ability to access patients' homes, which may not be wheelchair accessible. Sufficient hearing, eyesight, and physical dexterity for patient assessments and care Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
