Nurses will assist the Registered Nurse in executing the established plan of care, including performing specialized procedures under physician's orders and contributing to the evaluation of patient care plans. Responsibilities also include teaching patients self-care techniques, preparing clinical progress notes, and assisting with patient rehabilitation.
Candidates must have graduated from an accredited School of Practical Nursing and be licensed as an LPN in the State of Ohio, with a current CPR (BLS) certification required. One or more years of home health experience is preferred, and a criminal background check is mandatory.

This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rate Mileage reimbursement for any company travel Medical/Dental/Vision Insurance/Supplemental insurance (begins immediately upon hire) Company 401K with 4% company matching
Assist the RN in carrying out the plan of care Assist the RN in performing specialized procedures as directed by the RN under physician's orders Assist with the preparation, implementation, and continuing evaluation of the patient care plan Help teach the patient appropriate self-care techniques Prepare clinical progress notes Assist with the rehabilitation of patients according to the medical care plan All other duties as assigned
Graduation from an accredited School of Practical Nursing Must be licensed in the State of Ohio as a LPN One or more years in home health experience is preferred Must have a criminal background check Must have a current CPR (BLS) certification
Valid driver's license required Ability to access patients' homes, which may not be wheelchair accessible. Sufficient hearing, eyesight, and physical dexterity for patient assessments and care Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
