Provide community-based assessments and interventions to promote independence and reduce reliance on care packages. Manage a varied caseload, ensuring timely and effective outcomes.
Experienced community therapist, ideally with a background in Adult Social Care. HCPC Registration and relevant Occupational Therapy qualifications are required.
You will be expected to manage a caseload each week, conducting face-to-face visits and assessments across the county, ensuring individuals receive the right support to maintain independence and wellbeing.
