Care Manager I (RN) : Care Management

The Care Manager I (RN) coordinates patient needs across the continuum of care, collaborating with physicians, nurses, and community agencies to arrange appropriate care and develop comprehensive plans. This role involves reviewing assessments, providing psychodynamic intervention and crisis counseling, educating patients and families, and documenting all pertinent information as a mandated reporter.

Candidates must fulfill mandatory stroke education requirements and, for the RN position, possess a Registered Nurse license. A Bachelor's degree in Nursing (BSN) is required, although Magnet facilities allow for enrollment within one year and attainment within two years of hire.
