Advance Practice Provider (Nurse Practitioner or Physician Assistant) - Full-Time Clinical Team Care Model Role - PC and Internal Medicine (Floater)

Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced Nurse Practitioner or Physician Assistant to join our Primary Care and Internal Medicine team in a unique Full-Time role that will cover both clinics. In this role, you will spend approximately 50% of your time providing direct patient care and 50% supporting the provider team through clinical coordination and workflow support. Unlike traditional primary care roles, you will not be responsible for building or managing your own patient panel, allowing you to focus on excellent care, teamwork, and clinical excellence.

About the Role

As an Advanced Practice Provider at FHMG, you will function as a clinician, consultant, educator, and collaborative partner to physician colleagues. You will help coordinate high-quality, patient-centered care while supporting practice efficiency, population health initiatives, and quality improvement efforts.

You will provide comprehensive evaluation and management services to adult and geriatric patients, grounded in evidence-based medicine and aligned with national quality standards such as Patient-Centered Medical Home (PCMH) principles.

• Key Responsibilities
• Provide high-quality, evidence-based care to adult and geriatric patients, including history taking, physical exams, diagnosis, treatment planning, and patient education
• Spend approximately 50% of time in direct patient care and 50% supporting physician colleagues and clinical workflows (no independent patient panel)
• Order and interpret appropriate labs, imaging, and referrals; perform procedures within scope of practice
