The Nurse Care Manager will serve adult clients with severe mental illness or substance use disorders, providing services that include medication reconciliation and coordinating care with primary care providers. Responsibilities also involve conducting nursing assessments, providing wellness services, and offering follow-up coordination after hospital or ER visits.
Candidates must have completed a Registered Nursing (RN) Program and possess a current RN license in the State of Oklahoma, along with a BSN degree. This role is classified as 'safety-sensitive' under Oklahoma law, though medical marijuana license possession will not automatically exclude an applicant.
Requires completion of Registered Nursing (RN) Program, current licenses as an RN in the State of Oklahoma and BSN.
