The Physical Therapist is responsible for evaluating patients, developing and implementing individualized plans of care, and providing skilled physical therapy services in the home setting to improve mobility and function. Essential duties include coordinating care, educating patients and families, assessing equipment needs, and maintaining compliant clinical documentation.
Candidates must be graduates of an accredited Physical Therapy program and hold a current Physical Therapist license in the state of practice, with a minimum of one year of experience, preferably in home health. Strong clinical assessment, organizational, time-management, and communication skills are required, along with the ability to work independently.
