The Care Coordinator will be responsible for carrying out care coordination and patient navigation, which includes connecting patients with internal and external resources, assisting with referrals, and obtaining prior authorizations. This role also involves scheduling appointments, advocating for patients, and collaborating with various internal staff and providers regarding patient needs.
A high school diploma or equivalent is required, though an associate's degree is preferred, along with experience in community health or patient resource coordination. Candidates must possess excellent communication, problem-solving, clerical, and data entry skills, and must be able to read, write, and speak English, with Spanish being preferred.
