The Social Services Assistant coordinates and documents psychosocial services for residents throughout their care journey, including admission, stay, and discharge. This role involves assisting residents and families with adjustment to placement and the plan of care while adhering to federal and state regulations.
Candidates must have experience in a healthcare or long-term care setting and the ability to work effectively with elderly individuals who are physically, mentally, or emotionally ill. Strong skills in documentation, organization, prioritization, and maintaining confidentiality are essential.

Under the direction of the Social Services Director, our Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations, and our policies and procedures. You will assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility. You will also participate in resident and family council meetings as requested, and support Admissions and assist with discharge planning services.
