The POD Pharmacist develops and implements the MTM program, performing written member medication reconciliations and drug therapy action plans for all members, communicating these to clinical staff and providers. This role involves conducting telephonic consultations with providers regarding necessary medication changes and speaking with members about usage concerns.
Candidates must possess a Bachelor's degree in pharmacy or a PharmD, along with an active pharmacist license residing in the state of the position. A minimum of 3 years of active practice experience as a pharmacist is required, preferably in managed care, ambulatory care, or a clinical setting.
Position: Pod pharmacist
The POD Pharmacist helps develop and implement Monogram’s medication therapy management (MTM) program which will ensure appropriate use of medication management resources. The POD Pharmacist will perform timely written member medication reconciliations and drug therapy action plans for all Monogram members and communicate them to Monogram clinical staff and treating nephrologists, primary care physicians and other providers. He or she conducts telephonic consultations with medical providers when medication changes appear necessary and speaks with members about medication use concerns. The pharmacist will work collaboratively with clinical (e.g. nurse and social worker) care manager operations personnel to integrate medication management into Monogram’s overall care model and to ensure that field clinicians are executing on MTM policy and procedures.
