The Nurse Practitioner will provide professional medical services, maintain a patient panel, and coordinate care in collaboration with the care team, often leading the patient-centered medical home efforts with a physician. Key duties also involve establishing patient rapport, prescribing treatment and medication, promoting self-management, and providing health education.
Candidates must have graduated from an accredited Nurse Practitioner advanced degree program and should ideally possess two years of experience in an ambulatory care setting. Required credentials include an active Certified Nurse Practitioner license from Arizona, a current DEA Certificate, CPR certification, and a Fingerprint Clearance Card.
