The therapist will provide individual and group therapy to 4–5 adult clients daily in community settings, creating individualized treatment plans aligned with Assertive Community Treatment best practices. Responsibilities also include participating in daily multidisciplinary team meetings and maintaining accurate documentation in the EMR.
Applicants must hold a provisional or full North Carolina licensure as an LCSW/A, LCMHC/A, or LMFT/A, and possess a valid driver's license and reliable transportation. Community-based work experience is preferred, along with comfort in meeting clients in their own environments.

You’ll support adults with severe and persistent mental health needs by providing therapy directly where they live and thrive: in their homes, neighborhoods, and preferred community settings.
