The LPN will provide direct nursing care to residents, including medication administration and wound care. They will also observe and report changes in resident conditions and collaborate with the healthcare team.
Candidates must have a current, unencumbered LPN license and strong communication skills. The role requires the ability to make independent decisions and work effectively in a team environment.
· Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring. · Observe, assess, and report changes in resident condition to physicians and the nursing team. · Assist in the development and modification of resident care plans. · Document resident care accurately and thoroughly in electronic health records. · Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care. · Participate in facility surveys and quality improvement initiatives. · Maintain a safe and therapeutic environment for residents.
· Current, unencumbered LPN license. · Ability to make independent decisions and exercise sound judgment. · Strong communication and interpersonal skills, with the ability to interact tactfully with residents, families, and staff. · Knowledge of nursing and medical practices, procedures, and long-term care regulations. · Ability to work effectively in a team environment and supervise other personnel as needed. · Patience, compassion, and a positive attitude. · Ability to lift, push, pull, and transport residents with assistance as needed. Working Conditions: · Fast-paced environment with frequent interruptions. · Exposure to infectious waste, diseases, and hazardous chemicals (PPE provided). · May require working beyond normal hours, including weekends and holidays.
