The RN Care Coordinator oversees the progression of care and discharge planning for identified patients, ensuring quality of care and cost-effective outcomes while adhering to regulations. This role emphasizes communication and collaboration with various departments, insurers, and post-acute providers to achieve optimal patient outcomes.
Candidates must be graduates of an Accredited School of Nursing with a Diploma or Associate Degree (BSN preferred) and hold an active RN License. A minimum of two years of acute hospital clinical experience or a Master's degree in Case Management/Nursing is required, along with at least five years of total nursing experience.

Ability to observe changes in patient behavior, deescalate aggressive or agitated behavior, and anticipate need for further intervention. De-escalation training, preferred. De-escalation required if sitting for at risk for harm to self or others within 30 days of hire or transfer Ability to read, write, speak, and understand English. Basic computer skills High School diploma or GED required BLS with the American Heart Association Required Physical Requirements - Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)
