The Social Worker will educate clients and families on managing severe mental illness and achieving individual goals, which includes conducting daily home visits and creating individualized treatment plans. Responsibilities also involve completing assessments, developing goals for medication/housing/employment, providing evidence-based treatment, and connecting clients to community resources.
A Master’s Degree is required for this role, with a preference for CASAC, LMSW, LCSW, or LMHC certifications. Candidates must possess at least one year of experience working with the severe and persistent mental illness (SPMI) population and hold a valid, clean New York State Driver’s License.

Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program, which provides services to individuals who are diagnosed with severe mental illnesses and whose needs have not been met by more traditional service delivery approaches.
Travel is required using the agency vehicle.

Master’s Degree required.
CASAC, LMSW, LCSW, LMHC preferred.
1 year of experience working with the severe and persistent mental illness (SPMI) population required.
Ability to work independently and as part of a multi-disciplinary team required.
Excellent interpersonal and communication skills, both verbal and written required.
Proficient computer skills, including Microsoft Office and Electronic Health Records/Documentation, required.
Ability to multitask and prioritize in a fast-paced environment.
Valid and clean New York State Driver’s License required.
Sitting and standing for extended periods of time, as well as moderate lifting.
Must be able to go up and down stairs.
