The Home Health Registered Nurse will provide comprehensive skilled nursing care, including initial evaluations and ongoing OASIS assessments, while managing an entire caseload from admission to discharge. Responsibilities also involve educating patients and families on self-care techniques and coordinating care with physicians and multidisciplinary teams.
Candidates must be graduates of an accredited nursing school with a current State License as a Registered Nurse and a minimum of one year of experience in a clinical care setting. A reliable vehicle with valid insurance is required, as is the willingness to travel daily for home visits.
