The Senior Case Manager performs initial intake assessments, develops individualized service plans based on resident needs, and provides regular outreach and support, often involving direct, in-person engagement. Responsibilities also include advocating for residents regarding housing issues, assisting with benefit applications, and facilitating therapeutic/psycho-educational groups.
Senior Case Manager qualifications require an MA/MS with at least 2 years of experience, or a BA/BS with at least 4 years of experience, alongside knowledge of local social services for underserved populations. Candidates must possess strong interpersonal, charting, and professional writing skills, with the ability to manage crises and demonstrate cultural competency; Spanish language skills are desirable and offer a premium.
