The RN Case Manager will provide skilled nursing services to patients in their homes under a physician's plan of care, which includes developing plans of care with measurable outcomes in collaboration with the patient and family. Responsibilities also involve educating patients and assessing their holistic health care needs, demonstrating competency in assessment and technical skills, and utilizing knowledge of pain control and symptom management.
Candidates must possess a current Registered Nurse (RN) license in the State of Maine and a current CPR card, along with one year of recent clinical nursing experience. A reliable, insured car is also required, and ANA certification in Home Health or Community Health Nursing is encouraged.

Develop plans of care with realistic/measurable outcomes in collaboration with the patient/family. Provide education and promote patient’s rights and responsibilities for informed decision-making in the plan of care. Assess and identify the patient’s holistic health care needs. Demonstrate competency in assessment and technical skills. Possess and utilize knowledge of pain control & symptom management. What You’ll Bring: Current R.N. license in the State of Maine.
