The Advanced Practice Provider functions independently to manage complex acute, critical, and chronically ill perioperative patients, including performing age-appropriate history and physicals and ordering/interpreting diagnostic tests. Essential functions also involve prescribing treatments, implementing interventions like serving in a first assisting role during surgery, and facilitating patient transitions between healthcare settings.
Candidates must be graduates of an accredited school of nursing and an accredited Nurse Practitioner Program, holding current Advanced Practice Registered Nurse (APRN) Licensure. Required certifications include current BLS and ACLS provider cards, with PALS required as per department needs, alongside strong computer skills in Word, Excel, and PowerPoint.
Primary Care | Outpatient | Value-Based Care
Practice primary care the way it was meant to be practiced.
We are seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our primary care clinic focused on value-based, relationship-driven care. This role is ideal for an APP who believes great primary care is built on time, trust, prevention, and long-term outcomes—not rushed visits and volume-driven metrics.
In our clinic, APPs are trusted primary care providers with their own patient panels, supported by interdisciplinary teams and a care model designed to keep patients healthy, engaged, and out of the hospital.
Compensation & value-Based incentives
We believe high-quality primary care should be supported by compensation that reflects clinical expertise, continuity of care, and patient outcomes—not volume alone.
For experienced APPs, incentives reflect independent panel management, longitudinal patient outcomes, and contribution to population health goals.
For newer or early-career APPs, the compensation model emphasizes stability, structured onboarding, mentorship, and a gradual ramp-up of panel and incentive expectations.
Our goal is to align compensation with meaningful primary care work—supporting providers who take the time to deliver comprehensive, patient-centered care.
