The primary function of the Social Worker is to assess the psychosocial symptoms of the patient and family, developing outcomes to alleviate identified symptoms. This role also involves providing necessary psychoeducation, supportive counseling, and casework interventions to meet these outcomes.
The role requires the Social Worker to act as an expert within the core interdisciplinary group, focusing on psychosocial support for patients and families. The individual must commit to professionalism, collaboration, teamwork, and continuous growth while embracing innovation in hospice care.
