The Intake Coordinator manages the admissions process to ensure a comfortable experience for patients and a smooth process for staff and agencies by reviewing referrals and providing clinical assessments for appropriate placement. This role involves direct collaboration with facility admissions, marketing liaisons, physicians, hospital staff, and managed care organizations regarding facility levels of care and referral priorities.
Candidates must be skilled communicators, directors, and motivators who can effectively organize and prioritize multiple tasks while utilizing local and facility resources. Required qualifications include an LPN/LVN, RN, or Therapist license, a minimum of two years of experience in long-term care, and demonstrated knowledge of Medicare, Medicaid, and Insurance, along with reliable transportation.

Wage Commensuration The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
Review all referrals in a timely manner from hospitals, physicians and other key referral sources and providing a clinical assessment for appropriate placement in facility in the region Work directly with facility Admissions, Marketing Liaisons, physicians, physician groups and physician organizations, key hospital staff, managed care organizations, appropriate external and internal case managers Exemplify a working knowledge of the facility levels of care provided by the network, special programs and designated network referral priorities Read and interpret pre-admission assessments (the completed clinical field evaluation), communicate with Fundamental client facilities as needed Qualifications & Requirements Skilled communicator, director and motivator Able to organize and prioritize many tasks effectively Ability to use local and facility resources in the execution of job responsibilities LPN/LVN, RN or Therapist (clinician) with appropriate licensing required 2+ years in long-term care required Demonstrated knowledge of Medicare, Medicaid and Insurance required Reliable transportation required
