The Social Worker will provide services to individuals with severe mental illnesses, focusing on educating clients about mental illness, teaching coping skills, developing relapse prevention plans, and conducting daily home visits. Responsibilities also include writing treatment plans, conducting assessments, corresponding with managed care companies, and providing evidenced-based practice treatment.
Candidates must possess a Master's Degree and hold an LMSW, LCSW, or LMHC license, along with a minimum of one year of experience working with the severe and persistent mental illness population. A valid and clean NYS Driver's License and proficiency in computer skills, including EHR documentation, are also mandatory.

Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program, which provides services to individuals who are diagnosed with severe mental illnesses and whose needs have not been met by more traditional service delivery approaches.
Travel is required using the agency vehicle.

Master’s Degree required.
LMSW, LCSW, or LMHC required.
Minimum of 1 year of experience working with the severe and persistent mental illness (SPMI) population is required.
Proficient computer skills, including Microsoft Office and Electronic Health Records/documentation are required.
Ability to work independently and as part of a multi-disciplinary team is required.
Excellent interpersonal and organizational skills are required.
Strong communication skills, both verbal and written, are required.
Ability to multitask and prioritize in a fast-paced environment is required.
Valid and clean NYS Driver’s License required.
This position requires sitting for long periods of time and travelling to conduct home visits.
