The coordinator is responsible for scheduling branch field clinicians and maintaining proper care coordination by communicating with field staff, patients, physicians, referral sources, and caregivers. They must also manage the on-call notebook and hospitalization logs to ensure stakeholder communication.
Candidates must possess a high school diploma or equivalent and meet specific clinical or experience criteria, such as being a licensed nurse with clinical experience or having at least one year of home health/hospice/pediatric experience within the last 24 months. A valid driver's license and dependable transportation are also required for most locations.
Schedule patients to branch field clinicians. Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care. Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
