The Hospice Medical Social Worker will provide essential psychosocial support to terminally ill patients and their families, conducting comprehensive psychosocial assessments and offering crisis intervention. Key duties include facilitating advance care planning, providing grief counseling, coordinating resources, and actively participating in interdisciplinary team meetings to address emotional and social needs.
Required qualifications include a Master's or doctoral degree from a CSWE-accredited school of social work and a current State License as a Master's Social Worker (MSW/LMSW/LCSW). Candidates must also possess a minimum of 1 year of experience as a licensed Master's Social Worker and have reliable transportation for travel.

Pay Range $66,000—$71,000 USD
Employment is contingent upon the successful completion of a background check. Screening is completed by a third-party administrator, the Michigan Long-Term Care Partnership, and is performed in compliance with the Fair Credit Report Act.
