The role involves providing psychosocial assessment, individual/family therapy, and discharge/aftercare planning to patients and families in an outpatient setting, utilizing recovery-oriented services and evidenced-based treatments. This position also serves as the primary contact for patient/family education and acts as a liaison to community programs to enhance aftercare plans.
Candidates must possess a Master's degree in social work, counseling, or a related field, along with licensure as an LCSW, LMFT, or LPCC. A minimum of two years of recent relevant experience is required, along with knowledge of various treatment principles and strong organizational and communication skills.

Possess written and verbal communications skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical people.
Well-developed time management and organizational skills, including the ability to prioritize assignments and work within standardized operating procedures and scientific methods to achieve objectives and meet deadline.
General knowledge of computer applications, such as Microsoft Office Suite (Word, Excel and Outlook), electronic health records (EHR).
